A medical provider network (MPN) is specific to the state of California. MPNs are a listing of healthcare providers that will offer medical assistance to injured employees. "Self-insured employers or workers' compensation insurers," among other types of employers, according to the California Department of Industrial Relations website, are eligible to apply for an MPN. After reading and understanding the rules regarding MPN registration, an employer can apply -- and obtain approval -- through the CA Department of Industrial Relations.

      Visit the CA Department of Industrial Relations website. Click on the "Workers' Comp" tab in the main menu and select the "Medical Provider Networks" link.

      Click the "Answers to frequently asked questions about medical provider networks (MPNs)" link to find the rules and guidelines for applying. Familiarize yourself with the guidelines before moving on.

      Scroll down to the "How do I apply for an MPN?" question and open the "Cover Page for Medical Provider Network Application" link. Download and print the page. Complete the page, including tax identification and other specified information.

      Click on the "Find a Form" tab on the left sidebar menu. Scroll down to the "Medical provider network (MPN) forms" section and download the appropriate application. Submit the application and cover page to: CA Department of Industrial Relations, Division of Workers' Compensation, MPN Applications, P.O. Box 71010, Oakland, CA 94612. Allow two to four weeks for processing and approval. Contact the CA Department of Industrial Relations, Division of Workers' Compensation, for answers to approval questions, at: 800-736-7401. Allow 60 days for an official written response on the status of your approval.
