Colorado birth certificates can be located through the vital records office.

The Colorado Department of Public Health and Environment handles all requests for vital records, including birth certificates. Birth certificates are not considered public records in Colorado. You must have proof of a tangible interest in obtaining the birth certificate or a direct, familial relationship with the registrant. Fees to fulfill the request do apply.

Instructions

      Visit the Colorado Vital Records website (see Resources). Click on "Birth and Death Certificates" to the left.

      Download the online request form.

      Fill out the form. You will need to include your name, address, phone number, purpose of your request, relationship to the registrant and signature. You will also need to specify the registrant's name, date of birth, place of birth, father's name and mother's maiden name.

      Write out a check or money order in the amount of $17.75. Make it payable to "Vital Records." If paying by credit card, fill in your credit card information. A $9 convenience charge is issued on all credit card orders.

      Include proof of your relationship to the registrant with your application. Acceptable methods of proof include a copy of your birth certificate, marriage certificate or a government-issued identification card.

      Mail the application to the county in which the registrant was born or to the main Colorado vital records office. You may also deliver your application and payment in person.
