How do I Obtain a City of Phoenix Accident Report?
Accident reports may be requested after an accident for multiple reasons, such as insurance or court purposes. In any case, the city of Phoenix requires that anyone who requests an accident report is either a party involved in the accident or a relative of one of the parties involved. This rule is in place to protect the identity of the parties involved and keep the information private from anyone who was not involved.

Go to the city of Phoenix website.

Click on "Report search" at the menu on the left side.

Click "Accept" at the bottom of the page. This page explains that the person searching for the accident report must be a party involved or a relative.

Type in the incident number on the first section and the last name of one of the parties in the second section.

Click "Search." This will bring up the accident report.
