How to Fix a Disc Read Error on a PS2
A disc read error with a PlayStation 2 occurs when the PS2 has trouble reading discs or DVDs. Getting a disc read error does not have to be cause for panic, however. By determining whether the cause of the error is with a single disc or the console, the problem can often be fixed simply by cleaning the disc or the console itself.

Clean the disc. If it is only one disc that cannot be read, the disc is either scratched or dirty. Clean the disc using a small amount of rubbing alcohol, a lint-free cloth or cleaning fluid that can be purchased from video game and toy retailers.

Clean the console. Dust and hair can accumulate in the PS2 disc tray if it is left open. Use a can of compressed air to blow away anything that might be trapped inside.

Use a laser lens cleaner. This also helps to clean any grime that may have accumulated in the console.

Tips & Warnings
If none of the methods work, call the Sony support number and customer service will try to walk you through fixing the disc read error. If this doesn't work, you may have to send the unit in to get repaired.
Do not attempt to take apart the console if none of these steps work. You can further damage the system if you don't know what you are doing.
