Check your tie-in knot to make sure it's tied correctly. Also check to see that the knot is passed correctly through your harness.
Step2
Make sure the waist belt of the harness is doubled back through the buckle.
Step3
Take a second look to see that the belay device is properly rigged and that the gate of the locking carabiner, through which the rope is passed, is locked shut.
Step4
Check that the belayer is securely anchored to the ground or cliff, or that the climber is in a position where a fall won't jerk him or her out of position.

Do a verbal check with your partner before climbing. As you look him/her over, say aloud what you are checking, eg. "Harness doubled back, biner locked..." ect, and have them do th say for you. It makes sure that you remeber to do it everytime, and if your partner forgets to check something he should have, you'll probably notice.
