The Arizona Nutritional Assistance (formerly Food Stamps) Program helps to provide healthy food to low-income families with vulnerable adults and children. The program allows recipients to receive the food stamp benefits on an Electronic Benefits Transfer, or EBT, card. All food stamp recipients are eligible to receive an EBT card in Arizona. You have to apply for benefits and your eligibility is determined based on factors such as your income, certain expenses and your household size. You can turn in your completed application in person at your local Family Assistance Administration Office, online or by mail or fax.
Receive your benefits approval letter from the Arizona Family Assistance Administration Office. The letter will contain the approved amount and type of benefits you will receive. You should receive your letter within 30 days of your eligibility interview.
Get your Quest EBT card from the mailbox. Activate the card by calling the toll-free number on the card. Enter a four-digit PIN, when prompted. You will need the PIN number for every transaction. Use your Arizona EBT card at any store where Quest is accepted. It works like a credit or debit card to purchase eligible food items and non-food items, if you also receive cash benefits.
Receive your benefits on your EBT card every month during your eligibility, between the first and the 13th day of the month after your first month. The date you receive your benefits is determined by the first letter of your last name. For example, if your last name starts with "A" and "B," your benefits are loaded on your card every month on the first of the month during your eligibility.
