How to Legally Change the Date of My Birth

Among the information on your birth certificate will be the date on which you were born. However, sometimes the date of birth shown on the certificate is wrong. If you want to legally correct the date, you'll need to do so through the appropriate state government agency. Does this Spark an idea?

      Visit the Department of Vital Records website for the area where your birth certificate was issued to find out what you must submit to correct your birth certificate. For example, if your birth certificate was issued in New York state, you will visit the New York State Department of Health Vital Records section (see References). New York City residents will need to contact the New York City Department of Vital Records.

      Compile the materials needed to submit a birth certificate correction. Most states require a signed photocopy of your state identification, as well as a photocopy of your birth certificate and the original birth certificate. The state may also require that you submit proof of the error in the form of a letter from the hospital in which you were born.

      Download the application for correction of a birth certificate from the Department of Vital Records' website (see References). Fill out the form, including the information about the incorrect date of birth.

      Write a check or money order for the nonrefundable birth certificate correction fee. This fee varies from state to state. For example, in New York state it is $40.

      Place all of the materials compiled in Step 2 along with your application in an envelope, and send the envelope to the address shown on the bottom of the application. When your application has been processed, you will receive confirmation that your birth date has been legally changed.
