Download the application and print it.

ABC's Extreme Makeover Home Edition is accepting applications. Complete all sections of the application and answer all questions thoroughly. Questions include personal data regarding appearance, weight, height, family, income, household expenses, education, employment and other topics. The application is lengthy, consisting of 18 questions. Applications that are not complete will be disqualified.

Upload or mail a photo and/or video of yourself, your family and your house. This is a part of the application process.

Await contact from the network. Note that your photos and videos will become the property of the network. Due to the large numbers of applicants, it may take several weeks before you receive a response.
