How do I Apply for a DEA License in the State of Utah?
In order to administer, possess or prescribe a controlled substance in the United States, you must hold a Drug Enforcement Agency issued license. The state of Utah requires that medical professionals hold both a DEA license and a Utah controlled substance license to practice in Utah, according to the State of Utah Division of Occupational & Professional Licensing. If you're a first-time applicant for a DEA number, obtain the Utah controlled substance license prior to applying for the DEA license through the federal government to practice in Utah.

Obtain the Utah Controlled Substance application form through the Utah Division of Occupational & Professional Licensing website or in person at:

Fill out the application with your name, Social Security number, date of birth, address, driver's license number, phone number, email address, and all current and previous medical licensing information (in Utah and in other states). You also need an understanding of the Utah laws and rules pertaining to your practice, defined by the State of Utah Division of Occupational & Professional Licensing, to complete the form. Include the application fee of $100, as of August 2010, with the form.

Sign and submit the application in person to the location listed in Step 1. If mailing the application and fee, send via express mail or a delivery service. Send by U.S. Mail to:

Select your business category to begin the application. Categories include practitioners, manufacturers, pharmacies, distributors and treatment clinics. Choose your business activity (what you or your business does). These categories select the proper version of the application you complete and affect the fees involved.

Complete the application with your name, Social Security number, address and phone number (or business name, address, tax ID and phone number for a business). You also need your business activity and drug schedule information, as well as your Utah Controlled Substance License. In addition, have your background information pertaining to controlled substances available.

Submit the application electronically (if not in person) along with payment of application fees using a credit card. The DEA accepts Visa, MasterCard, American Express or Discover cards. Fees range from $184 to $2,293 (as of August 2010) depending on the business category and activity selected, as does the period of licensing (one to three years).

Confirm and correct all of your information before final submission. Print and retain a copy of the application for your records.

Application fees for both the Utah Controlled Substance License application and the application for a DEA license are nonrefundable once submitted.
According to the DEA application website, section 843(a)(4)(A) of Title 21, United States Code, states that any person who knowingly or intentionally furnishes false or fraudulent information in the application is subject to imprisonment for not more than four years, a fine of not more than $30,000, or both.
