How Do I Get a Handicap Parking Form in Florida?
In the state of Florida, people with a physical disability are entitled to receive a disabled parking permit. This permit will allow you to legally park in spaces that are reserved for disabled people. These spaces are typically closer to businesses and other establishments, and they're also typically close to ramps used by those in wheelchairs or people who find it difficult to climb stairs. Florida determines whether or not your disability requires a disabled parking permit largely based on the recommendation of your doctor. Before you can receive your permit, you need to fill out a form.

Make sure you qualify for a disabled person parking permit in the state of Florida before getting the form. To find out if you qualify, schedule an appointment with the doctor who treats your disability, such as your general physician, chiropractor, optometrist, osteopath, podiatrist or other specialist. You can also get this information from a registered nurse, physician's assistant or physical therapist. If the doctor or other medical professional is licensed in the state of Florida, he'll be able to tell you if you meet the requirements for a parking permit.

Click on "Application Form" and select "Retrieve form in pdf format." This will bring the form up on your screen.

Print the form by selecting "File" and then "Print." Once printed, you can fill out the form and return it to the State of Florida Department of Highway Safety and Motor Vehicles Division of Motor Vehicles.
