How to Write an Employee Corrective Action Form
The ideal employee follows instructions to the letter, is never late, puts in extra time and truly enjoys the job. Unfortunately, ideal employees are more the exception than the rule. If all employees were perfect, there would not be much need for managers. Managers are in place to ensure that an employee is doing the job properly and that business procedures and policies are followed. If an employee is having a problem, set up a meeting and write a corrective action form to help fix the problem. Consider more drastic measures, including termination, if the problem persists.

Place a header at the top of a blank document that has your business's logo. Title the form "Corrective Action Form" or a similar title.

Use section one for the employee's name, his job title, the date of the infraction and the supervisor's name.

Use section two to summarize the corrective action. Consider using check boxes or just use a line such as "Level of Corrective Action" with space to write in such measures as a verbal warning, suspension or termination.

Include space on the form to list the facts of the incident, the solution reached and any comments the employee has. Use tags such as "Facts," "Solution" and "Comments" with space provided to fill in the necessary information.

End the form with lines for the supervisor and the employee to sign and date. Print off several copies of the form.

Meet with the employee and use the form to fill in the required information. Place a copy of the form in the employee's file and give her a copy.
