How to File an Accident Report With a Public School
Children are very active. It is only a matter of time before one gets hurt on school property. Most public schools are required to fill out an accident report whenever any type of injury occurs on their property. This is true even if the injury is considered minor. Accident reports should also be filled out if a school employee gets hurt. Filling out an accident report is not difficult.

Take care of the student who is hurt first. It is also important to call the parents and let them know about the incident.

Get an accident report form from the school nurse. This should be done as soon as the child or employee has been attended to.

Fill out the injured student or employee's name. Then include his address and birth date. This information should be in the student's file. An employee can just relay this information.

List the location of the injury. If it happened on the monkey bars in the playground, list that. If it happened at the lunch table in the cafeteria, list that.

Document the injury. List what part of the body was injured, and in what way. Include whether the injury was minor, or of a more serious nature.

List the names of any students or school employees who witnessed the accident. You may want to include their versions of the accident.

Finish by writing down the exact time a parent was notified.

Make an extra copy of the accident report to send to the parents, or to give to the employee who was injured. Keep a copy in the student's file in case it is needed at a later time. You may also be required to give a copy of all accident reports to the principal or superintendent.

Tips & Warnings
Make sure you know where your school keeps its accident report forms before an accident occurs.
Do not wait to fill out the accident report. You will not remember as many of the details as when you fill it out immediately. You don't want a parent suing you and winning because you didn't document all of the information as soon as it happened.
