How to Get a New SSN Card
Virtually every American needs an SSN (Social Security Number) card. While a Social Security card is not something you carry with you at all times, you will likely need it if you want to get a job or apply for government assistance or benefits. Once you have an SSN card, though, you may find a need to replace it. You may lose it or need to make changes on it. To get a new Social Security card, gather information, find your documents, and file the correct form.

Visit the Social Security Administration website to find out what documentation you will need to get a new card. The electronic leaflet, "Your Social Security Number and Card" explains the different documents needed depending on your circumstances.

Gather enough documents from the list to prove your age, identity, and citizenship or immigration status. Birth certificates, identification cards, and U.S. passports are some of the most helpful documents.

Provide evidence for any changes that need to be made on your Social Security card. Submit a divorce, adoption, or marriage certificate if a name change is required, for instance. The Social Security Application Form explains how to change the date of birth, if there has been a mistake. Do this by filling in the wrong date, which is currently on your card information, on line 13. Put your actual date of birth on line 4 and support the correct date with evidence.

Locate and fill in a Social Security Form SS-5. Find a paper copy at your Social Security Office. Look online, if you prefer, at the special Social Security webpage where you can find the form and print it (See Reference 2).

Fill out the Form SS-5 completely, according the directions given. Mark the square "unknown" in spaces where that is allowed, if you need to do so. Remember to date the form and include your contact information. Sign your name.

Take the completed Form SS-5 and your documents to a Social Security Office or SSN Card Center to apply for your card. The Social Security Administration website has a page where you can input your zip code and find the nearest SSN Offices and Card Center. If you would rather, you may mail the Form SS-5 and your documents to the same Office or Card Center that serves your area. The documents will be returned to you.

Tips & Warnings
You can use copies of documents as long as they are certified by the agency that issued them.
Pay attention to your mail delivery from the time you turn in your application until you receive your new SSN card to make sure it does not get lost in the mail or stolen from your mailbox.
