If you are disabled or care for someone who is, you know how difficult it can be to find a parking space close enough for comfort. Whether you're new to Ohio or to driving entirely, the process to obtain a handicapped parking placard is straightforward. With the right forms, fees and proof, you can attain your placards in a matter of hours.

  • Doctor-signed prescription with date, name, disability and how long until you heal (if applicable)

  1. Preparation

    • 1

      Print the application for disability placards (BMV 4826) and fill out all applicable fields. (There is a link to this form in the Resources section.) If you do not have a printer, the BMV offers copies of this form. To have your application considered, you must be a disabled resident of Ohio.

      Have your doctor or health care provider write a prescription stating that you're applying for a disabled placard. This document must clearly contain your name, the date, reason for application, how long disabled placards will be needed and a signature from your provider. It must be an original; copies will not be accepted.

      Gather all applicable fees. As of February 2010, there is a $3.50 processing fee and an additional $3.50 charge if you need an extra placard.

    Getting Your Placards

      Decide whether you want to submit the application, fee and prescription in person or mail it instead. Submission in person will yield the placards quicker.

      Decide to pay by cash, check or money order. Money orders and checks should be made payable to Ohio Treasurer Kevin L. Boyce.

      To apply in person, take the paperwork and application fee to any Deputy Registrar license agency in Ohio.
