How to Create a Membership Form for a Fitness Center

A staple of any fitness center business is a membership form. You can choose to register customers on paper, online, or offer both options. Having each client fill out a membership form will not only help you keep organized, but allow you to provide personalized services based on each person's needs. It can also serve as your liability waiver form. Fortunately, creating a membership form for a fitness center on your own is simple and inexpensive.

      Open a new document in a word processing program. Insert the fitness company's logo at the top of the document, or type in the name of the fitness center.

      Type in the following headings, giving each its own line:

      Date
      Name
      Address
      City, State, Zip Code
      Date of Birth
      Do you have any medical conditions or recurring injuries?

      You can change the order of these headings if you'd like, although this order is standard on most fitness center forms.

      Create a section on the form that asks your clients about their goals. The section should be headed: "What are your fitness goals?" Options available are:

      Weight Loss
      Strength
      Flexibility
      Toning
      Muscle Building.

      Instruct your clients to circle as many options as apply.

      Add a section about payment methods. This is also where you will collect customers' credit card information. Make sure that you ask for the following:

      Name on credit card
      Billing address
      Credit card type
      Credit card number
      CVV or validation code

      Ask your clients to select their membership type, noting the price next to each option. For example, you can offer a monthly, six-month, or one-year membership, charging $12.95, $65.95, and $106.95 for each membership, respectively. What you charge will depend upon your location, and what you feel is just for the services you offer.

      If you offer personal training at your fitness center, ask the following question on the form: "Are you interested in personal training services?" This will give you an indication of which clients are interested in personal training, while preventing you from hassling those who are not.

      Add an injury liability statement, a statement about refunds and cancellations, and a statement about how often their credit card will be charged. You need to consult with an attorney to help develop these statements. You can make a separate injury liability waiver form.

      Conclude the membership form with signature lines for both your client and a fitness center administrator.

      At the very bottom of the form, add an administrative section labeled "For Office Use Only." Here you can leave room to write each client's membership number and other administrative details.
