How to Obtain a Utah Birth Certificate
Without a birth certificate it's difficult to get important items like a driver's license, a Social Security card or a passport. A birth certificate proves who you are and confirms that you're a citizen of the United States. The Utah Office of Vital Records and Statistics has maintained birth certificate records since 1905. It has an easy application process if you need to obtain a Utah birth certificate.

All Utah birth certificate requests must include an enlarged photocopy of your identification. Both front and back sides must be copied. Acceptable forms of ID include: Driver's license or state-issued identification, student ID, employment or military ID, passport, tribal ID card or a U.S. citizenship card.
If you don't have any of these, you'll need two secondary forms of ID. These include a paycheck stub or W-2 paperwork, voter registration or Social Security card, car title, hunting or fishing license, or any other legal documents that show your name and current address.

If you are going to mail or submit the application in person, download, print and complete the Birth Certificate Request form.

Submit the Birth Certificate Request Form and copies of your ID. It costs $18 for one certified copy of the birth certificate and $8 for each additional copy. Mail everything to: Office of Vital Records and Statistics, PO Box 141012, Salt Lake City, Utah 84114-1012. Make the check or money order to "Office of Vital Records and Statistics."
You can also take the paperwork to: 288 North 1460 West, Salt Lake City, Utah. Office hours are Monday through Thursday, 7 a.m.-6 p.m.
Online applications are accepted through a service called VitalCheck and may be charged an additional service fee.

Wait to receive the birth certificate. Mailed requests typically take seven to 10 business days to process. If you take your application to the Office of Vital Records before 5:30 p.m., you'll receive the birth certificate the same day. VitalCheck takes seven to 10 business days, or one business day if you choose expedited service.
