      Download the IRS Form 4506T (Request for Transcript of Tax Return). This is a .pdf that can be printed from your computer. Tax return transcripts can also be requested by phone at 1-800-908-9946.

      Fill out the IRS Form 4506T. Keep a copy for your records.

      Send the form to the address provided at the top of the .pdf file 4506T form. Allow two weeks for delivery.
