How to Print a Change of Address Form From the US Post Office

To print a change-of\-address form from the U.S. Post Office, you must go online and enter your address information. The website does not give you the option to print blank change-of-address forms. Submit the form online after printing if you wish. You can also print the form before submitting electronically and mail or deliver it by hand to the U.S. Post Office.

  1. Step 1

    Go to the www.usps.com home page and click on the Household tab. From the "Use these quick and easy online tools" dropdown menu in the "Conquer your to-do list" box near the top of the page, click on "Change your address when you move."

  2. Step 2

    Accept the Understanding and Privacy Statement by clicking the large orange arrow. Note that under the arrow, it states that a valid credit card account billing address matching the moving-from or moving-to address and a valid email address are required. However, it also states that if you do not have these, an option to print will appear.

  3. Step 3

    On the next page, choose permanent or temporary address change. Enter a starting day, month and year. Click the "continue" button. On the page to which you are taken next, choose individual, family or business address type. Click the "continue" button.

  4. Step 4

    Fill in your name, old address and new address. Do not enter an email. By not entering an email, you receive an error message that lets you choose to print. Click the "continue" button.

  5. Step 5

    When you click the "continue" button above, an error box takes you back to the previous screen to enter an email address. Take note of the message under the request that you enter an email address. It gives you a "print the change of address form" link to use if you do not have a valid email address. Click on that link rather than enter an email so that you are given the option to print.

  6. Step 6

    This link takes you to a preparer's information page. Enter your name into the preparer's name request box. Do not worry about the other boxes unless you wish to enter that information. They are not required. Click on the "continue" button.

  7. Step 7

    View the Official Change of Address form now on the screen and confirm that the information is correct. Complete the verification text box at the bottom of the page. Click "continue."

  8. Step 8

    On this page, click any catalog check boxes to have those catalogs forwarded to your new address. Click "no thanks" or confirm when finished checking for catalogs you wish to have forwarded. An offer for magazine subscriptions appears next. Click any you wish to receive or click "No thanks, only print address change form" to continue.

  9. Step 9

    The next page lets you know your form has been automatically sent to your printer. A button to view and print the page manually exists on this page as well. Click this to view the page prior to printing. Read the directions for delivering or mailing the change of address form to the post office. Print your form and deliver to the Post Office.
