How to Print a Change of Address Form From the US Post Office

Although the United States Postal Service website does not give you the option to print blank change of address forms, it does provide an electronic form you can use to request a change of address online. If you prefer not to pay the small fee for submitting change of address information online, the website generates a filled-in version of the form from the information you provide that you can then print and mail.

Instructions

      Go to the USPS homepage (see Resources) and from the Manage Your Mail drop-down menu click "Change Your Address."

      Review and accept the Privacy Act Statement by clicking the orange "Click Here to Begin" arrow.

      Specify whether the address change is permanent or temporary on the next page, and enter a starting day, month and year. Click the "Continue" button. On the page to which you are taken next, choose "Individual," "Family" or "Business" for the type of move and click the "Continue" button.

      Fill in your name, old address, new address and email address. Click the "Continue" button.

      Scroll down to the bottom of the Identity Verification page if you prefer not to pay to submit your form online. Click the "Print Your Request" link.

      Click the check box under "Please Verify the Mover's Information" and enter your name--and optionally phone number and email address--in the "Enter Preparer's Information" section. Click the "Continue" button. The form may print automatically, or a Print dialog box may appear.

      Click the link to view and print the form on the Ready to Print page if the form did not print automatically, and use your browser's print options to print the form.

      Follow the directions on the printed change of address form for delivering or mailing the form to the post office.
