2. Fill out the statement and file it with the county clerk.

3. Publish the statement with a general circulation newspaper in the county once a week for four consecutive weeks.

4. File an affidavit of publication with the county clerk within 30 days of publication.

5. Obtain the Assumed Name Certificate and file it with the secretary of state.
