Logon to Georgia's Department of Revenue, Motor Vehicle Division Web site and download an official application for a personalized license plate.

Print the application.

Read all instructions, including page two, before proceeding to the application portion. Keep all rules, regulations and policies in mind while filling out the application.

Return to the top of page one and begin filling out your application. Begin by typing or legibly printing the owner of the vehicle's full legal name, address, county of residence, daytime telephone number and email address.

Check the box of the type of the vehicle that will be using the personalized license plates.

Enter six different choices for your personalized license plate messages using capital alphanumeric character combinations. Bear in mind that you are not allowed more characters on a plate than is otherwise noted for each vehicle type. Symbols and punctuation are not allowed on personalized license plates.

Specify the meaning of each personalized license plate message. The department will not process your application without first making a determination on the appropriateness of your messages.

Give the owner of the vehicle's full legal name again.

Provide the owner of the vehicle's driver's license number and state of issue or a Georgia ID card number.

Certify your application by signing and dating it.

Find your local County Tax Commissioner's office.

Mail your completed application to your local office along with any applicable fees, payable to the Office of the Tax Commissioner. All cash payments must be made in person at the office.

Receive your personalized license plates.

Affix month and year tabs on your permanent personalized license plates upon receipt and use.

Tips & Warnings
If you move, be sure to contact your local County Tag Agent's Office and give them your new address. If you do not advise the office of your move, receipt of your personalized license plates may be complicated or delayed.
Be advised that if your plate is approved, it may undergo some changes during manufacturing. Certain characters' widths may cause the spacing specified to be reduced, eliminated or altered.
The address on your vechile's registration must be correct. Contact the MVD and make all changes to your address before placing your order.
It is very important that your application be legible to avoid misinterpretation of information and messages. Be sure to print legibly or use a typewriter with a widely recognized font.

The department will not honor requests for personalized license plates that contain prohibited characters, messages that negatively effect public safety, offensive or misleading messages, trademarks, or alphanumeric combinations that are already in use. Keep this in mind while choosing your plate message.
Personalized license plates cannot be manufactured as souvenirs. Do not submit an application unless your vehicle is currently and legally registered and you plan on using your personalized license plates on your vehicle.
One application for a personalized license plate needs to be filled out for each plate that you are requesting.
If Georgia's Department of Revenue approves your personalized license plate and later finds that your message is offensive or inappropriate, your personalized license plate may be recalled.
Blank or incomplete areas on your application will cause the application process for your personalized license plate to be delayed. Be sure to fill in all requested information completely and truthfully.
If your vehicle is sold, traded or its ownership transferred after the issuance of the personalized license plates, you must contact Georgia's Department of Revenue to report the transaction. You must also follow the appropriate guidelines to release the plate to the new owner, transfer the plate to your new vehicle or return the plate to the department. Be advised that additional paperwork may be required.
Once your order has been processed and paid for, all requests for changes, refunds and cancellations will be denied.
