Only the registered owner of the vehicle can apply for a copy of the title, and only the Florida Department of Highway Safety and Motor Vehicles is authorized to issue duplicate vehicle titles in the state. A duplicate vehicle title voids the original title.

      Download form HSMV 82101 from the Florida Department of Highway Safety and Motor Vehicles (DHSMV) website. This form is the application for a duplicate title.

      Take the completed form to a DHSMV office. You can find the nearest office location on the DHSMV website. Alternatively, you can mail the form to the DHSMV.

      Show proof of identification, such as a driver's license, and pay the duplicate title fee. As of May 2010, the fee is $75.25. If you need the title immediately, you can pay an additional $10 for fast service.
