Visit the NBC casting website to get the details for where to send your application if you believe you're tough enough to endure an encounter with Steve-O.

Decide which way you want to submit your application materials. Although applicants have the option to use email (drsteveocasting@bunim-murray.com), postal mail (Attn: Dr. Steve-O Casting, 6007 Sepulveda Blvd., Van Nuys, CA 91411), and the online application on the USA website, the "Dr. Steve-O" casting department prefers email submissions. If you opt for email or postal mail, save the completed application at USA in a printable format and copy and paste it into an email or print it out on paper for mailing.

Go to the USA website to access the "Dr. Steve-O" casting application.

Answer each of the questions on the application thoroughly and honestly, providing as many details as needed to help demonstrate why you would be an excellent guest to appear on "Dr. Steve-O."

Locate two photographs that were taken of you recently and transfer them to your computer for uploading with the completed application if you choose to submit it online. If you email the application, include the photographs as attachments. And, if you mail the application, include copies of the photographs in the envelope.

Submit the application in your preferred manner after proofreading your responses to catch spelling and grammatical errors.
