Hi, I'm Doctor Raphael Darvish with Skinpeccable Dermatology and Cosmetic Laser Center, here in Los Angeles, California. Today, I'm going to show you how to get rid of a cystic acne pimple. Our patient today, Jasmin, has a small pimple right there that she got this morning, and she wants it gone. There's many ways to treat an acne cyst, whether it be by topical or oral medicines. Some benzoyl peroxide products have been shown to be effective in controlling acne and acne cysts. And there are some devices out there, including a device called The Zeno, that works well to decrease the acne cyst. But really, the gold standard or really the most effective, home run treatment for an acne cyst is going to be a steroid injection. So, these are the materials that we're going to be using to do an acne cyst steroid injection, first they are steroid. This is MethylPrednisolone, 40 milligrams per milliliter. This is our one percent lidocaine that we will use to dilute the MethylPredinisolone with, our gloves, alcohol swabs, a 32 gauge needle, a couple larger needles and the syringe that we will use to pull it up. This is some gauze that we will use afterwards, just in case the patient has any bleeding. Step one, I will put my gloves on, and so, you gotta pick a set of gloves that are your size. Mine are medium. And you want to definitely be comfortable. You don't want anything too big or too small. Step two, you want to take your alcohol swab and wipe off the tops of both the steroid and the diluent. In this case, lidocaine, one percent. Step three, we want to open our syringe, and then open our needle that we're going to use to pull up our steroid. So, here we go. The following step, we pull up the steroid. We're going to pull up point one of the steroid. So, that's right 'til about there. Put the needle on after that, that's step five. Take this needle on; I don't want to cross contaminate my lidocaine with any drops of the steroid. So, step six, I've put on a new needle to pull up my lidocaine. So, we'll pull up right around to1.0 or so. And we don't want to use such a large needle for our injection, so what we will do is take that needle off, and replace it with a thirty two gauge needle. You want to push up the product so that the liquid is at the tip. Step nine, we're going to alcohol the acne cyst. Step ten, is we're going to actually do the injection. So, go ahead and lean your head back, Jas. So, here we go, you're injecting right there, perfect. So, the lesion has been injected, and I've just put a small drop the steroid in there. And I'm just going to hold the pressure, which I guess will be step eleven, to prevent any bleeding, for around 30 to 60 seconds, will stop that bleeding. She'll go home, and in around six to 18 hours, this lesion will go flat; and by tomorrow, she really shouldn't notice it very much at all.
