"Hi! I'm Dr. Darren Adamson from Argosy University in Salt Lake City. Here to tell you a little bit about filling out a daycare application. Clearly as a parent you're very concerned about insuring that your child has every opportunity at the daycare to thrive. It starts with filling out the application well and properly. First thing that you want to do is review all the information on the application, make sure that you understand what's being asked for. It's important to be really clear about the information that the daycare center is asking you to provide. The second thing is, don't leave anything blank. If there isn't any answer that applies to the particular question or the information being requested, simply put NA, meaning, not applicable. It's also very important to use in black or blue pen so that the ink is clearly legible and that the daycare center can make out what it is that you're trying to tell them. That brings up also in showing that what you put on the daycare application is legible. Make sure that you're writing not in cursive but writing in block lettering so that the daycare center can have the most accurate information possible. It's also important when you're filling out a daycare application to ensure that you read it prior to turning it in. Fill it out, read what you've put in there, making sure that the answers are accurate and put in information that would be very unique to your child, not just simply the information being asked for. Most daycare applications have a place for you to put comments or additional information about your child. It's important to help the daycare center get to know your child before he/she starts there. So make sure you tell them everything you can about that's unique about your child."
