"Hi my name is Veronica Baragas with my wiggles and giggles dot com. And I like to share with you some of the child daycare forms. So there's a few essential forms that you are going to need in enrolling your child into any daycare. So first would be your care giver and parent agreement. Basically this is just going to cover you know what family is going to be enrolling their child, who there child is, kind of talks about what contracts are going to be and what papers are going to receive so. And basically you are going to sign off and say, you know I acknowledge that this person is going to be taking care of you know my son or daughter what not. One of the others is admission information, you are basically going to want to get you know name, address, date of birth, emergency contacts, what doctors to go to, things like that. Of course all the parents information and emergency contact information and also on the bottom it just basically gives authorization for medical attention basically, in case of an emergency or whether you need to you know give out medication things like that. This is one of the standards that you have to provide for this date. Discipline and guidance policy, basically it's just going to say you know this is what I would do in case of needing to discipline your child or what not. Things that I will not be doing and this is under state regulations as well, that this is provided to the parents and signed to know that all parties involve know exactly what to expect. I provide a yearly calendar of days that I'm going to be closed. Or you know just kind of stating here that you allow yourself x amounts of vacation days or x amount of sick days or what to, you know, professional development days things like that. So this is good for parents to know, that way they can plan ahead. And then one of the other ones that you can also do which is optional is just kind of do one large sheet or contract I guess that the parents would keep on hand to kind of give all of your policy information, all of your philosophy information, any kind of state regulations that you need to provide or what not. And just make sure that they have that on hand so at any time they can refer back to one paper that they've turned in to you or what not. So definitely keep up with your child care forms and look for the department of family protective services to give you a list of those required forms when you register with the state."
