"You want to change your name on your social security card. Hi, I'm Robert Todd, and I'm here to answer how do you go about changing your name on a social security card? Well, one of the first things you're going to do is complete an application for the social security card. It's a form SS dash 5. It can be obtained from the Social Security Administration. You're going to have to show the Social Security Administration proof of your citizenship or your immigration status. You're going to have to show proof of the actual legal name change, and you're going to have to have some form of identity. Now, if you live in New York City, or Las Vegas, or Orlando, or Orlando, Florida, or Phoenix, Arizona, you're going to have to do this in person. Otherwise, you can mail copies of these documents to the social security agency. Now remember, when you are changing your social security card, your name on the social security card, you're going to have to complete this form SS dash 5.
