"Hello, my name is Luis Estrada and I am a software developer. In this video I'll show you how to create a form on a web page with FrontPage. FrontPage is a registered trademark of Microsoft, a company to which I have no affiliation. The easiest way to include a form is using the form page wizard. Press on the new file button and select page. From the general tab select the form wizard and press ok. The wizard will automate the process of creating input fields for data entered by visitors through our website. This data can be stored as a text file, a web page, or sent as an email. Press next, now press on the add button to insert a list of questions for the users. From the menu select the best option depending on the kind of information that you want to gather from your users. You can chose between personal, contact, account, product and order information. Also you can ask for boolean values like yes or no, dates, ranges, numbers etc. For this case we will make it simple and select the contact information option. Notice that the prompt question is provided for us. You can edit it or leave the default one. Press next, choose what data fields you want your users to fill, press next. You can add more data inputs by pressing on the add button again. Also you can modify and delete any of the inputs. Press next, now we have the presentation options. You can chose to present the input fields as paragraphs or as a numbered bulletin or definition list. The second option creates a table of contents. You should use this option only if the form is really long. The last option uses a table to hold the input fields. Choose this option because it gives a more organized look for the page. Press next, select the way that data from users should be stored, either as a web page, or as a text file, or use a custom script. A script is programming code used to handle the input data. For example, to store it in a database, to perform calculations with it, to send it by email etc. Keep in mind that for any of the storing options to work the front page extensions should be installed in the web server where you will host your website. Contact your hosting provider to ask about the availability of these extensions and for support on how to use them. Finally, click on the finish button.
