One option is that you may be someone who's been on antibiotics, maybe from another dermatologist or from your internist, and your acne's just getting worse and worse. In that case, the dermatologist may decide to take a culture to see if you have what's called gram-negative folliculitis, where possibly, using the broad-spectrum tetracyclines for a period of time has caused some bacterial overgrowth of a different kind of bacteria called gram-negative bacteria.
