I just made this in a simple word processor. You can make all the forms that you need. There is some great books available as well on punctuality and business. Now one of the sad things that happens is a lot of entertainers just aren't good business people. But if you keep good records, save all your receipts, make sure you're on time for your gigs, you're are going to find that people are anxious to hire you, they are anxious to work with you and they're are anxious to pay you to come make balloons for their next party or their next event."
