"Now we're ready to complete the 1040EZ form and in this example we're going to start with the "Label" and we're going to do this for both an example that's a married couple, and an example of just someone filing single. So, our single person is "James B. Jones", and we would put his name, first name and initial and last name, then we want his Social Security number up here and his address and city and state. If he did have a label from the IRS we would just simply paste the label over that. Now, if you want to contribute $3 to the Presidential Election Campaign Fund, you would just check the single view box. If it's a couple filing now, say James was married to Sarah Jones, the only difference would be is that you'll put the name in here under "Spouse's Name" and then you would put her Social Security number under "Spouse's Social Security number. And, the same address and city and state. And she would also check the "Spouse" box if she wanted to contribute $3 to the Presidential Election Campaign."
