What Documents Do I File With a 1040X?
The 1040X form is provided by the Internal Revenue Service to allow you to amend a return from the previous year. This may be because you made a mistake and did not include some income or loss or you may have forgotten to include dependents on your previously filed paperwork. Do not file it if you are simply telling the IRS that you made a mistake in math---they will find the mistake on their own---or to claim penalties or interest (use Form 843 for that). Also, do not file this form for an injured spouse. For that, the IRS says to file form 8379. If you still need to file a 1040X, there are several items that the IRS says you must include with your newly filed paperwork.

The Tax Return You Are Amending

This may seem superfluous; however, you do need to provide the IRS with a copy of the return you are amending. Even though they may have copies on file somewhere in their offices, they will require that you provide a new copy with this filing as well.
Supporting Documentation from the Previous Tax Return

The IRS also requires that you include all "supporting forms, schedules and worksheets" you completed with the previous tax return, and not simply send in the base return alone.

Whether you are claiming a loss or paying taxes you owe, the IRS also requires that the newly filled out 1040X include all supporting forms and worksheets to show why you needed to file the form.
Form 8913, Credit for Federal Telephone Excise Tax Paid

According to the IRS, if you are claiming a credit for telephone excise tax, you must include Form 8913 with Form 1040X explaining the credit.
Form 5405

If you are using the 1040X to claim a first time home credit, the IRS requires that you include Form 5405 explaining this credit.
Form W-2c and Form W-3c

If filing Form 1040X for the purposes of claiming corrected household employment taxes (i.e., if you employ a maid or other household help and pay taxes on their wages) you must also include form W-2c and W-3c to explain the new status of your employees.
