ACCESS Program

Also for persons with disabilities, the ACCESS program has the same eligibility requirements as the Mainstream program, with the added stipulation that the applicant currently reside in a nursing home or other assisted-living residence, is under 61 years old, and wishes to transition to a more independent living situation.
