How to Put a Form on Facebook
Putting a form on your Facebook page requires three major steps, which include creating and hosting the form on your servers, setting up a Facebook canvas and adding a tab to your Facebook page. Once completed, your Facebook page visitors can fill out your form with their information and it'll be forwarded to you. Using a form is a quick way to gather feedback or build your email marketing list.

Create your form and upload the file to your hosting account's server. You have the choice of creating your form in HTML, Javascript or PHP. You could include almost anything on your form and depends on your needs. For example, if you are looking to capture email addresses, you would put a text field box for the person's name and their email address. Once you have uploaded the form online you should make sure you can access the file by its Web address and that it works as designed.

Navigate to developers.facebook.com, click the "Apps" option on the top menu bar.and sign in to your Facebook account to set up an application. If this is your first time using the Facebook Developers site, then you are asked to confirm your identity by adding a credit card or verifying your mobile phone number. There is no cost associated with using the Facebook Developers site.

Click the "Create New App" button at the top of the screen. Enter the name of your app, select the language that the application is written in and agree to the terms of use for creating apps. Click the "Continue" button.

Navigate to the "On Facebook" section from the left sidebar and go to the "Page Tabs" subsection. Fill in the "Tab Name," "Tab URL" and "Secure Tab URL" fields. The "Tab URL" and "Secure Tab URL" should point to the Web address where your form is located. Click "Save Changes" when done.

Navigate to the left column and click the "View App Profile Page" link. On the next page, click the "Add to My Page" link in the profile's left sidebar.

Click the "Add to Page" button for the page you want to add the form to. Click the "Close" button when done. Your form is now added as an app tab to your Facebook page and is visible to allow visitors.
