How to Not Have Your Name in the LDS Register
The Church of Jesus Christ of Latter Day Saints (LDS), also commonly referred to as the Mormon Church, keeps detailed records of all active, inactive and disfellowshipped members of their church. These records include information about baptisms, tithing, priesthood ordination and, in some cases, personal information about you and your family. If you have been inactive for years, or are considering leaving the church, it can be a challenge removing your name from the church registry. Doing so requires a bit of legwork on your part because information about being removed from the registry is not made readily available to church members.

Contact your local LDS bishop. If you have any sort of rapport with the bishop, he may be able to expedite the name resignation process. If you're unsure of who your local bishop is, contact any local LDS church branch. They should be able to provide you with this information.

Write a letter to the church membership records office headquartered in Salt Lake City, UT. You can word or structure the letter any way you like. However, the letter should include your full name, current address, date of birth, baptismal date and any priesthood ordinations, as well as any temple ordinances performed. This information will help to verify that you are who you say you are. It's also wise to include a statement about your reasons for wanting your records removed.

Send an e-mail to the church membership records office. Include the same information provided in Step 2. Send the e-mail to Greg Dodge at dodgegw@ldschurch.org. As of July 2011, Dodge oversees all church membership records in the U.S.

Tips & Warnings
There is no standard waiting period for church record removal. The waiting period can be anywhere from 30 to 90 days. If you don't get a response in 40 days, send the letter or e-mail again. You may have to be persistent in doing this every month until your records are officially removed from the registry.
