How to Create a Questionnaire in Google to Be Filled In

Google Docs, a productivity application provided with a free Google account, includes word processing, spreadsheet, presentation, forms and drawing software. The sharing options allow either privacy, sharing among certain people, or sharing with anyone on the Internet. You can create a questionnaire using Google Docs and distribute it by email, or put it on your blog or website, then view the results in various formats.

      Sign in to your Google account, click the "More" tab on the top navigation bar, and choose "Documents" from the drop-down menu. You might already have "Documents" as a tab on the navigation bar -- if so, click the tab.

      Click "Create new" and choose "Form" from the drop-down menu.

      Enter a title for the form in the first text box, and then a description or instructions in the second text box.

      Click on the "Theme" button located above the form editor, choose a theme to view a sample, and then click "Apply." Otherwise, keep the default "Plain" theme.

      Click "Sample Question 1," and then click on the pencil icon to edit. Type the question in the "Question Title" text box. Type instructions for the question in the "Help Text" text box. If the question is self-explanatory and no instructions are needed, leave that box blank.

      Choose a "Question type" from the drop-down menu. From here, you can choose among various options including multiple choice, check boxes and text.

      Click "Add item" from the upper-left menu bar to continue inserting questions, then click "Save" to finish.

      Close the form editor to view the spreadsheet version of the form. Click the "Share" button, then the "Change" link to choose visibility options. Click "Learn more about visibility" to decide which setting is appropriate for your questionnaire.

      Click "Form" from the spreadsheets edit menu, then choose "Send form" to distribute the questionnaire by email, or choose "Embed form in a webpage" to copy the HTML code for your blog or website.

      Review the answers by opening the form from your Google Docs homepage, where the answers appear in spreadsheet form. View the results in a summary and graph format by clicking "Form" and choosing "Show summary of responses."
