How to Legally Change Your Place of Birth on Your Passport
Passports that have been issued with incorrect information can be reissued by the U.S. Department of State. If you determine that your place of birth is incorrect on your passport you can submit an application, along with a certified copy of your birth certificate, to have it corrected. The State Department will make the correction free of charge.

Go to the travel.state.gov website for the U.S. Department of State, International Travel. Click on the tab near the top of the screen entitled "Passports." Click on "Forms" on the left side of the screen.

Click on Form DS-5504: Application for a U.S. Passport - Name Change, Data Correction, and Limited Passport Replacement. This will be near the bottom of the screen.

Complete the form by clicking on "Complete Online & Print" under Step 1. Follow the instructions on each screen and print the completed application.

Sign and date the application on the first page. Mail the application, along with a certified birth certificate and your current passport, to the address listed on page two of the form.

Receive your new passport, free of charge, by priority or first class mail. Processing time is four to six weeks. Overnight and expedited services are available for a fee.

Tips & Warnings
The form can also be completed by hand.
You will need to include a new passport photo if your passport was issued more than 90 days ago.
