1
Obtain an empty Grand Soul Gem. Among other places in the game, they are available for purchase in the mage's guild in any town.
-
2
Wait for the Revenant Shade to randomly show up. It will be in one of three random locations and can be pinpointed due to its distinct purple light in the sky. This occurs every eighth (in-game) day. The three locations are Fort Istirus, Fort Linchal and Dark Fissure. At each of these locations where the Revenant Shade appears, there is an altar. When it appears, travel to the location and approach the altar.

3
Place the empty Grand Soul Gem onto the altar. Approach the altar and open your inventory menu. Scroll to the empty Grand Soul Gem and select "Use."
-
4
Cast a Soultrap spell or use a Soultrap scroll on the altar. Align the crosshairs on the altar, turning the crosshairs to an eye. Cast the Soultrap spell or use the Soultrap scroll. This transforms the Grand Soul Gem into a Black Soul Gem. Collect the gem from the altar.
