How to File a Change of Address With the Western District Court of Missouri
Any person dealing with the Western District Court of Missouri for any reason must notify the court of a change of address when moving. The court must have the correct address on file at all times when you are involved in a case. Obtain a change of address form from the Western District Court of Missouri. Some change of address forms are available for you to download and print online; however, others must be obtained in person at the court.

Provide your name, Social Security number, case number (depending on the type of case you are involved in), your previous address and your new address on the form.

Sign the change of address form and file it with the district court by delivering it to the courthouse and giving it to a clerk.
