How to Order From an LDS Storehouse
As part of its mission to provide for the poor and needy, the Church of Jesus Christ of Latter-Day Saints operates a welfare program which provides food and clothing for church members when they're temporarily unable to provide for themselves. If you're a Bishop, you can order supplies from a storehouse for a church member or family by completing and submitting a couple of forms.

Complete a Needs and Resource Analysis form. State the name of the member seeking supplies, his or her ward, stake, and contact information. Include the names and ages of any dependent family members. Write down the immediate needs of the person or family and identify potential contributors and resources in the community.

Consult with the member about his or her plan to move from temporary dependency to self-reliance. State on the form any work assignments that the member and his or her family members are able to accept. Have the member sign the form.

Complete a Bishop's Order for Commodities - Food and Supplies form. State the name and age of each individual that will be receiving supplies along with the date of their enrollment in the program. Make your selections from the list of food and clothing, as agreed upon with the member. Sign and date the form and submit it to order the needed supplies.
