How to Anonymously Report to OSHA

The Occupational Safety and Health Administration is authorized by Congress to act on behalf of American workers to ensure employees are working under safe conditions. If you are an employee of a company that you feel has unsafe working conditions, you may submit a complaint to OSHA. OSHA investigates the complaint and if requested properly, keeps the complaint anonymous to your employer.

Instructions

      Visit the OSHA website and click the "Workers" tab. Under "How To Contact OSHA," located in the upper-left corner of the page, click "Complaint."

      Scroll down and click the "Complaint Form" link under the "Online" section or the "Download and Fax/Mail" section. If you prefer to fill out and submit the form online, click the "Complaint Form" link from the "Online" section. If you prefer to print out the paperwork and mail or fax it in, click the "Complaint Form" link from the "Download and Fax/Mail" section.

      Fill out the form and answer all the questions required.

      Check the box that states "Do NOT reveal my name to my Employer."

      Submit your complaint form as desired by Internet, fax or mail.

Tips & Warnings

  • OSHA respects your privacy and desire to stay anonymous and does not reveal your name to your employer. However, you still must include your name and contact information to OSHA.
