How to Transfer a CNA License to Ohio

Most states, including Ohio, have "reciprocity" agreements to transfer CNA licenses from one state to another. The Ohio Nurse Registry also extends a four month grace period, which means that a CNA can work for four months before being listed on the Ohio Nurse Registry. Once you have applied for transfer to the Ohio Nurse Registry, you can check your status online.

Step 1

Contact the Nurse Aid Registry in the state you reside and request an "Application for Enrollment By Reciprocity" to transfer a CNA license to another state.

Contact the Ohio Department of Health Nurse Registry by phone at 800-582-5908 (or 614-752-9500) and ask to apply for CNA nursing reciprocity.

Fax or mail the following information to the Ohio Department of Health Nurse Registry:

Copy of your state issued drivers license Copy of your Social Security card Copy of your current CNA license Full legal name Complete mailing address Contact phone number
