List the name and birthday of your spouse and any children, as applicable.

Provide a list of any properties you have an interest or liens on, such as a Deed, Contract for Deed or Certificate of Title. The list should include the location details, including the county.

Take the forms to a notary public. Sign in two places witnessed by notary.

Mail the forms to the district court of Red Lake County or drop them off at the administrator's office. Include the applicable filing fees.

Wait for the forms to be returned with approval for the name change.
