If you need a duplicate Social Security card, you can get one from your local branch of the Social Security office in Illinois. Download form SS-5 from the Social Security website. Complete the application by filling in the information requested.

Obtain a certified copy of your marriage or divorce decrees that contain your full name if you're changing your name on your duplicate card. Photocopies are not allowed. A current Illinois driver's license with a photo is required to prove your identify. Take the application and required documents to the Illinois Social Security branch closest to your location. Present your application and verifying documents to the representative. The representative will verify your documents and identity before submitting the request for a duplicate card.

If you can't make it to your local Social Security office, you can send the application and original documents and identification to the nearest Social Security office. Your original documents will be returned to you through the mail.
If you don't have access to a printer, you can obtain an application from the nearest Social Security office in Illinois.
You are only allowed 10 duplicate Social Security cards in a lifetime in order to replace stolen or lost cards. If the duplicate card is for a name change or change in citizenship, there is no limit.
