How to Change a Last Name After a Divorce in Boone County, Missouri
Divorce is a trying time for both men and women, and once your marriage has been declared dissolved by a court in Boone County, Missouri, you are entitled to restore your last name back to your maiden name (as the ex-wife) should you so desire. Schedule a day to make a driving trip to Columbia, Missouri. When the day arrives, gather together the items listed in "Things You Need" and commit them to a folder that can be closed to prevent losing any of the documentation.

Drive to the Boone County Clerk's office, located at 801 E. Walnut St. in Columbia, Missouri. Take the documentation and black pen along with you and go to the front desk of the clerk.

Explain to the clerk that you have been divorced, that the process has been finalized, and that you would like to file the necessary papers to revert your name back to your maiden name. The county clerk may ask to see a copy of the finalized divorce for inspection, so provide the documentation to her when asked. She will provide you with a packet of forms that the state of Missouri requires you complete and submit. Take the forms to a suitable location and fill them out.

Fill out all of the state-required forms completely, leaving none of the blanks empty. If something doesn't apply to you specifically, enter "N/A" or "Not Applicable" in the blank. When entering the case number from the "Dissolution of Marriage" declaration (from your finalized divorce packet), double-check to make sure you entered each of the numbers and/or letters correctly.

Sign and date the forms once completed and return them to the Boone County clerk's office. Submit them for processing and you will be told how long it will take for the matter to become legal public record for both Boone County and the state of Missouri. Refrain from using your maiden name until you have received notice from the state of Missouri that your name change has been accepted and activated.
