How to Request a Post Box Installation From the US Post Office
In most cases, the United States Postal Service (USPS) requires citizens to purchase their own mailboxes that are approved by the USPS. Customers can get detailed specifications of those mailboxes and how they must be erected in order to ensure the mail carrier can open the box and slide in mail, all the while not leaving his/her mail carrier truck. If you have mail needs that cannot be served adequately by a mailbox and regular carrier service, then request a post office box from the USPS.

Visit your local post office and ask for "PS Form 1093."

Present two forms of valid identification for yourself and begin filling out the form.

List any other people you wish to allow to open the box (e.g. a business partner, spouse or children) along with two forms of valid identification for each person.

Submit the form to a post office staffer.

Wait for the USPS to notify you of the location and number of your post office box.
