How to Change Your Name in Jefferson Parish, Louisiana

Submit a "Petition to Change Name" form at the parish courthouse.

A name is not always a permanent thing. In Jefferson Parish, La., you can petition to change the name you were given at birth, or one you adopted upon getting married. Virtually anyone (except convicted felons) can legally change his or her name, but must go through the courts to do so. You can alter a given name you always resented, or revert to your maiden name after divorce. The process does not require a lawyer.

Instructions

1. Obtain a "Petition to Change Name" form from the Jefferson Parish courthouse.

2. Fill out the required information in the form, including birth name, desired new name, reasons for requesting a change, and current residence.

3. Submit the form along with any requisite filing fees.

4. Wait for the judge to review your petition. If the name change is granted, you are responsible for notifying banks, creditors, the passport office, the Social Security Administration, and other official entities and organizations of your new name.
