You can order additional copies of your birth certificate from the Spokane Regional Health District in Spokane, WA.

Birth certificates are issued to babies born in Spokane hospitals upon birth. Parents are asked to provide the information that goes on the certificate to hospital staff. Staff then sends a birth certificate form to the state and a birth certificate is created; new parents receive a certified copy of the child's birth certificate in the mail after it is recorded and filed. If you need an additional copy of your own or your child's birth certificate, you can order one from the Spokane Regional Health District.

Instructions

      Print a copy of the birth certificate application from the Spokane Regional Health District website (see Resources). You can also pick a copy up or request one mailed to you at the Spokane Regional Health District office in Spokane.

      Complete the birth certificate application form by supplying the information listed on the birth certificate you wish to acquire, your name and address and the number of copies you need.

      Write a check or obtain a money order payable to the Spokane Regional Health District in the amount of $20 per copy for the birth certificate you are requesting. Mail your application and check to:

      Your birth certificate application is processed and your certificate is mailed to you within 24 hours of receiving your application.

      Deliver your application to the Spokane Regional Health District between the hours of 8 a.m. and 4:45 p.m., Monday to Friday at:

      In person, you can pay with a check, money order, cash or credit card, and you will receive your birth certificate within minutes of submitting your application.

    Over the Phone

      Call the Spokane Regional Health District at 509-324-1524 to request a copy of your birth certificate over the phone.

      Provide the clerk at the SRHD with the information listed on the birth certificate as well as your name and address.

      Provide your credit card number to pay for your order. Your birth certificate is mailed within 24 hours.

      Order a birth certificate online by going to the Spokane Regional Health District website's birth and death certificates page (see Resources).

      Fill out the online application by providing all of the information located on the birth certificate. Provide your name and address.

      Use your credit card to pay for your order. You must enter the credit card number, expiration date and the security code located on the back of the card. Submit your order. Your birth certificate is mailed to you as soon as your application is processed.
