The Federal Motor Carrier Safety Administration (FMCSA) requires that commercial truck drivers undergo a medical exam that is commonly know as the Department of Transportation (DOT) physical. The driver fills out the driver's information and health history section of the report, and the medical examiner completes the rest of the report detailing the results of the physical exam. If you're interested in knowing the results of your exam, you may obtain a copy of the report for your records.
Request a copy of your DOT medical examination report from the medical examiner, nurse or receptionist at the medical office where you had the exam. You will be given a medical release form. The medical release form gives the medical examiner permission to release your medical report.
Read over the medical release form. Fill out the requested information, such as your contact information, type of records you're requesting and a reason for requesting the records. Make sure you specify that you're requesting a copy of your DOT medical examination report. Sign and date the form.
Submit the form to the receptionist. You will either receive a copy of your DOT medical examination report while you're in the doctor's office or it will be mailed to you.
