How to Write an Affidavit Sample

An affidavit is a sworn statement before a court of law. This statement carries the responsibility of full honesty and disclosure. You are subject to penalty if you lie on an affidavit form. As such, you should draft your affidavit honestly. The process for creating an affidavit can be completed with some standard procedures.

Instructions

1. Compose the header. You can start the affidavit by stating "Affidavit of (insert your name)."

2. State your facts. Below your header, list the relevant facts. The first fact should give your name, age, job and residence. Start fact No. 2 with a relevant fact about the issue.

3. Finish the affidavit by composing the footer. Type in something akin to "(Insert your name), being first duly sworn on oath according to law, deposes and states that she has read the affidavit of (Insert your name), that the matters stated herein are true to the best of her information, knowledge and belief." Take the affidavit to a notary public to notarize the form.Sign the affidavit at this time.
