Military police file incident reports on everything from car accidents to criminal activities.

Military police, like civilian police, are required to submit incident reports on infractions occurring on military installations. Incident reports are governed by Army Regulation 190-45, "Law Enforcement Reporting." Military police reports are completed on Department of the Army Form 3975 in accordance with AR 190-45. When more than one incident has taken place, an addendum to the DA Form 3975 must be filed on a DA Form 3975-1. Concurrent reports are assigned the same report number and are submitted to the Provost Marshal together.

      Obtain blank DA-3975s through your desk sergeant at the beginning of your shift.

      Complete the identification box on DA Form 3975. Include the police report number given to you by your desk sergeant and the date you are completing the report. If the "TO" box does not already contain your commander's information, add it. Write your department's information in the "FROM" box. The "THRU" box should only be used when forwarding the DA-3975 through a third party.

      Complete Section 1 by checking the appropriate boxes. If the report involves a criminal offense that resulted in arrest; for example, check "Criminal" in Box 1 and write an X in the appropriate box in Box 5a.

      Fill out Section 2 as completely as possible. Include the number of parties involved -- both offenders and victims -- in Boxes 1b and 1c. Refer to the location codes at the bottom of DA-3975 to complete Box 1d. For example, if the Class VI store was robbed, write the number "17" in Box 1d.

      If the report is not about criminal activity, leave Boxes 3 and 5 blank.

      Write the number of pages your report includes at the bottom on the lines available for this information. The bottom right corner of the DA-3975 says "Page __ of __." For example, if you are also using one DA-3975-1, the original page should read "Page 1 of 2." If you are using three DA-3975-1s, the original page should read "Page 1 of 4," while each subsequent page carries the appropriate number. The last page must read "Page 4 of 4."
